A doctor’s excuse note is a “legit” document that excuses you from work, school, sports, etc. Look at the list below to see the (5) five rudimentary requirements of a doctor’s excuse note:
1. Authentic hospital or clinic logo
2. Name, address & phone of doctor and clinic or hospital
3. The date the note was created.
4. Patient’s name and illness
5. Signature of doctor
The doctors excuse needs to state that the patient was seen.
The note can also detail how long the patient needs to remain away from the workplace, or school, or whatever activity the doctor’s excuse is for. However, there should not be too much information provided as to what the ailments consist of because of doctor/patient confidentiality. (ie appropriate language: walking pneumonia – patient to remain in bed for 7 days. – inappropriate: walking pneumonia causing patient much distress and anguish as well as physical pain – patient to remain in bed for 7 days while receiving antibiotic treatment on 3-hour intervals).
